Cortisol/Adrenal Fatigue Checklist

Rate the occurrence of the following symptoms. 5 being common
or something that happens regularly and 1 being an occasional symptom.

__ Constantly tired
__ Frequent headaches
__ Nausea or stomach in knots
__ Trouble sleeping
__ Difficulty concentrating
__ Loss of interest in things you use to find interesting, including sex
__ Depressed
__ Increased PMS
__ Increased pain
__ Memory loss
__ Prone to accidents
__ Overwhelmed, can’t get things done on time

30-40 points – significant adrenal fatigue
20-29 points – mild adrenal stress
less than 20 – low to moderate levels of adrenal stress
